Semaglutide
Class: GLP-1 receptor agonist
Main labeled uses: Type 2 diabetes in adults; selected cardiovascular and kidney risk-reduction indications
Route: Weekly injection or daily tablet
Age: Adults
Diabetes brand. Injection max is 2 mg weekly; the kidney indication applies to the injection only. Tablets are taken once daily on an empty stomach with up to 4 oz of plain water, at least 30 minutes before food, drinks or other oral medicines.
Tirzepatide
Class: GIP and GLP-1 receptor agonist
Main labeled uses: Type 2 diabetes; cardiovascular risk reduction in adults with type 2 diabetes at high risk
Route: Weekly injection
Age: Adults and children 10 years and older (cardiovascular use: adults)
Diabetes brand. Adult maximum is 15 mg weekly; pediatric maximum is 10 mg weekly.
Semaglutide

Main labeled uses: Chronic weight management; selected cardiovascular, pediatric obesity, and MASH indications
Route: Weekly injection or daily tablet
Age: Adults; injection also for obesity age 12 and older
Weight-management brand. Injection and tablets are both labeled for cardiovascular risk reduction; MASH and age-12-and-older use are injection only. Injection maintenance is 2.4 mg weekly (Wegovy HD 7.2 mg for selected adults); tablet maintenance is 25 mg daily.
Tirzepatide
Class: GIP and GLP-1 receptor agonist
Main labeled uses: Chronic weight management and moderate-to-severe obstructive sleep apnea in adults with obesity
Route: Weekly injection
Age: Adults
Weight-management brand. Maximum dose is 15 mg weekly.
All four carry a boxed warning concerning thyroid C-cell tumors observed in rodents and are contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2.
Other important risks may include pancreatitis, gallbladder disease, severe gastrointestinal reactions, dehydration-related kidney injury, hypoglycemia with insulin or sulfonylureas, and aspiration risk around anesthesia.
Ozempic and Wegovy both contain semaglutide. Mounjaro and Zepbound both contain tirzepatide. The Wegovy and Zepbound labels state that using them with another product containing the same ingredient, or with any other GLP-1 receptor agonist, is not recommended. Don’t double up on the same ingredient.
